Social Media Authorization:
I, the undersigned, allow Camden Pet Hospital to publish pictures of my pet which may include the health information/story for educational purposes: (e.g., information relating to the diagnosis, treatment and health care services provided or to be provided to my pet) on Facebook, our website, and/or other promotional material.
The following information about me will not be disclosed: health/dental history, date of birth, address, phone number, social security number, etc. Only my pet’s name, photo/video, and treatment described above will be disclosed and only with my permission.
Camden Pet Hospital’s Code of Conduct:
Camden Pet Hospital's guiding principles are to provide the best possible care to our patients and clients in an ethical and legal manner. We seek to be an advocate for the animals throughout their entire lives and are here to be compassionate, understanding and thorough in our care by focusing on education and enhancing the human-animal bond.
One of the most important criteria for the delivery of veterinary medical care from veterinarians and staff members at this facility is a polite, effective, comfortable, and open avenue for communication. Additionally, shared confidence between client and doctor is an essential component of an effective veterinarian-client-patient relationship as is seeing a patient annually to provide services and best guidance.
Due to the many challenges we face as veterinary medical professionals, and our goal to treat every animal as if they were our own, we require a respectful manner and attitude when interacting with any staff members of Camden Pet Hospital. We will not tolerate any level of abusive behavior - raising voices, threatening gestures, inappropriate language, demeaning communication, etc.
Failure to meet these requirements will result in the termination of services with Camden Pet Hospital. We also encourage our clients to visit NOMV (Not One More Vet) to further educate themselves on the crisis in the veterinary medical field.
Financial Policy:
I assume all responsibility for all charges accrued for the care of my pet. I understand payment is due at the time of service. I understand Camden Pet Hospital does not offer payment plans or accept partial payments. I know an estimate of charges is available upon request. I understand if I accrue a balance and fail to pay it by the established due date, a collection fee of up to 75% of the total balance will be assessed. I understand prepayment may be required when Camden Pet Hospital deems appropriate (such as new client’s appointments, clients with a history of missed or late canceled appointments, or a history of unpaid balances).
